The company has been around for a long time and offers other web services such as website design. . The Medforward forms are very easy to create, allowing for Boolean questions that are dependent on previous answers. I think anyone with any skill level can create very detailed forms. But if you do not want to create the forms their support team is very helpful and responsive and will make it for you. We have created smart online forms that ask questions dependent on previous answers, therefore allowing for very detailed intake forms. We require these intake forms of all our patients. The forms allow for capturing credit card information in a secure way. Also they allow for true signatures. All these features are also mobile-friendly. There is a dashboard view of completed forms and they have a full suite of reports for running analytics on the types of answers received, if that interests you. The most important part is that Medforward forms are very affordable with transparent volume-dependent pricing. They can also create custom forms for you if you don't have the time or inclination to make them yourself. Also they can create easy to use fillable PDF forms - either directly on the PDF form or as a separate set of questions but later superimpose the answers back into the original PDF. I have been using Medforward forms for more than 8 years. I do wish they would make an app so that we Can also use the forms in our iPad kiosk. Patients can enter everything on their own device - WITHOUT registering for a portal, WITHOUT a printer, AND we no longer have problems with reading handwriting. Whenever we schedule an appointment, we tell patients about completing their forms online in advance of that appointment - allowing us to request pertinent medical records, resolve insurance issues, etc, all before the appointment even occurs. Our workflow is so much smoother and we can dedicate more time to patient care once the person reaches our office. I love that it is all HIPAA compliant and we can capture electronic signatures. I also can't say enough about the responsiveness of Medforward staff. They are always prompt to answer questions and I always talk with a human being. Even though I have never met any of the staff in person, I feel like I know them. This is hugely important since, no matter how good a piece of software is, it does no good if the customer service is not equally stellar. > > **改善点**: There really isn't anything that I don't like. However, it would be wonderful if there was a more comprehensive way to allow certain data items (i.e. phone numbers and SSN) to be entered in any format the user chooses and then they would be converted to a standard format of our choice. We no longer have to worry about several different versions of forms floating around since the current ones are always available on our website. Making it easier for patients to do forms without registering for a portal produces higher rates of completion BEFORE the appointment and then more focused attention on the patient AT the appointment.
